Centre and Corona Virus Working Practice

We are happy to be welcoming face to face sessions back in the Matlock Therapy Centre. This form is the basis for good working practice in the centre buy is not a substitute for your own covid policy that should be in place between you and your clients.

Please fill in this form to submit that you have read and agree to the following:

5) Clients and practitioners are to be made aware that they should not attend face to face contact if they have experienced possible Covid symptoms or been in contact with anyone that has tested positive or displaying symptoms.

6) I will find the QR Code for the track and trace app as I enter the building. If you or your client are not happy to use it then you must ensure that you have a record of their contact details and the date and time at which you saw them as this information may be requested by track and trace. They must also be aware that they need to inform you if they test positve after seeing you.

7) I will arrange to meet clients at the door so that I can take them straight through to the session. This will mean clients are aware not to arrive early and I have enough time between sessions to make sure the room has been cleaned and aired and to meet the next client. I am aware that the waiting room is only to be used by agreement with the centre manager.

8) After each session I will take responsability for the cleanliness of the room to ensure the safety of my next client.

9) In my sessions I will ensure 2m social distancing and that only the people needed directly for that session are present, I will also ensure that if possible and practical the room will be ventilated.

10) I will ask my clients to bring their own water / refreshments and understand if I need to provide water to use a disposable cup from the kitchen

11) If I have any concerns about the buildings cleanliness that I cannot easily address I will notify the centre manager

12) When your name ahs been added to the board in the shared kitchen you will make sure you mark yourself in and out on this board.

13) You agree to only use the centre with prior agreement with the centre manager, most easily done through whatsapp on 07855767565 or through email

14) When you have finished your sessions please leave the door marked as free and turn your light out. If you are the last person in the building you must turn all lights off and lock the front door on the way out. If you cannot be sure the building is empty you must at least take the front door off the latch so it cannot be opened from the outside.

15) The number of hours you have used the centre for should be emailed through to at the end of each month so you can be invoiced accurately.

15) You agree to only work at the centre if you have your own indemnity insurance and will follow all the needed prodcedures of your professional regulations.
